Pennsylvania Health Information Exchange
The Pennsylvania Health Information Exchange – or PHIX – will provide the information technology (IT) architecture to support statewide interoperable electronic health records and electronic prescribing by sharing data captured at the point of care in a physician office or hospital. Currently, most doctor’s offices, hospitals, laboratories, and pharmacies all have their own separate information systems. With a Health Information Exchange, those entities will be able to share information with various health care providers and other authorized parties for treatment purposes.

PHIX was created through Executive Order by Governor Edward G. Rendell in March of 2008.

Chronic Care
The Chronic Care Management, Reimbursement and Cost Containment Commission was created through Executive Order by Governor Edward G. Rendell in May 2007 to improve how Pennsylvanians with chronic disease receive health care in the future. This initiative is part of the Governor’s Prescription for Pennsylvania health care reform plan.

The Chronic Care Management , Reimbursement and Cost Containment Commission is responsible for developing the process to effectively manage chronic disease across the state. About 78 percent of all health care costs in Pennsylvania can be traced to 20 percent of all patients -- those with chronic diseases. We cannot reduce the occurrence and cost of chronic diseases without aggressively addressing prevention, detection and treatment in a comprehensive, pro-active way.

The Commission will work to design the informational, technological and reimbursement infrastructure needed to support implementation of the Chronic Care Model throughout Pennsylvania, which will improve quality outcomes and cost effective treatments for patients with chronic diseases. Implementation of the Chronic Care Model will be through collaboratives, either statewide and/or regionally. The goal of the Commission is to draft a strategic plan by the end of 2007 for implementing the proposed changes.
